  • A copy of this paperwork must accompany each INDIVIDUAL pre-drawn blood sample.

Human Blood Sample Guidelines:

PLEASE FEEL FREE TO HAVE YOUR OWN DOCTOR, NURSE FRIEND, OR COWORKER DRAW YOUR SAMPLE.

  1. Sample must be less than 20 days old on November 9th, 2019.
  2. Draw at least three ml’s of whole blood.
  3. Let clot, spin and pour off serum into a non-additive empty plastic vial. LABEL VIAL
  4. Freeze serum immediately. Wrapping vial in paper towels for protection while traveling.
  5. Put vial in its own Ziploc bag.
  6. Put paperwork in another Ziploc bag and insert serum sample bag in with paperwork; protecting the paperwork  from sample and visa-versa.
  7. Put FROZEN sample with paperwork in a cooler with ice packs and drop off at any of the locations listed below after October 17, 2019.

 

If we do not have your legible paperwork, money and sample by November 9, 2019 at 5pm it will not be going. 

If you need blood drawn we will be offering a clinic on November 9, 2019 in East Montpelier. Send in your application ASAP.  

NO BLOOD DRAWS AT ANY OF THESE LOCATIONS ONLY FROZEN SERUM DROP OFF POINTS.
